What this measure does

This is not a hospital bill. It is a staffing bill, and it exists because a national law raised a hospital's bed capacity without giving it the people to run the beds.

Republic Act No. 11870 gave the Dr. Serapio B. Montaner Jr., Al-Haj Memorial Hospital an authorised capacity of 100 beds with effect from 29 June 2022. This measure requires the manpower requirement of the upgraded hospital to be met, directs that corresponding plantilla positions be proposed, and appropriates funds for them.

Why it was proposed

The premise is that an upgrade in name is not an upgrade in fact. A hospital authorised for 100 beds but staffed for a smaller facility cannot use the capacity it has been granted — the beds exist and the doctors, nurses and support staff to attend them do not.

The bill's answer is the narrowest possible: leave the national designation alone, and legislate the plantilla positions and the money for them. It is the only measure in this registry whose entire subject is the staff rather than the structure.

Because the cost is permanent salaries rather than a one-off build, it is the kind of commitment a single initial appropriation cannot discharge.

What to notice

This bill is the diagnosis the rest of the hospital cluster needs. Twenty-two other measures in this registry create or enlarge hospitals; this one addresses what happens afterwards, and the answer it implies is that nothing does. A national statute authorised 100 beds in June 2022, and a year later a member of the regional Parliament had to file a bill to get the posts created.

Read against BAA 80, which upgraded the same hospital from 25 to 100 beds in the region's own law, the picture is of 2 legislatures raising a bed count and neither supplying the complement. And read against Bill No. 146, where a 50-bed designation under Republic Act No. 11330 ran on a 10-bed budget for years, the pattern is established rather than anecdotal: in this region the designation arrives and the staffing does not.

What the bill gets right is the instrument. Plantilla positions are the difference between a hospital that has staff and one that hires on job orders — contractual posts without tenure or benefits, which is what the Lamitan bill documents as the reality for 6 of its 10 physicians. Legislating the positions rather than appropriating a lump sum for salaries is the only way to make the staffing permanent.

It also shows the limit of what a regional Parliament can do about a national act. The bed capacity came from Republic Act No. 11870 and stays there; the region can only supply what the national law left out. That is a narrow and honest reading of the region's own authority, and a more modest one than several other bills in this registry take.
